By accessing or using the Pharmacompound web application, you agree to be bound by these Terms and Conditions.

You are responsible for safeguarding your password and for any activities or actions under your password. You must notify us immediately upon becoming aware of any breach of security or unauthorized use of your account.

The Pharmacompound app and all its contents are the property of Pharmacompound and are protected by intellectual property laws.

In no event shall Pharmacompound, nor its directors, employees, partners, agents, suppliers, or affiliates, be liable for any indirect, incidental, special, consequential, or punitive damages, including without limitation, loss of profits, data, use, goodwill, or other intangible losses.

The Pharmacompound app is provided “as is” and “as available”.

Subscribers may terminate their subscriptions to Pharmacompound’s service at any time by following the instructions provided by logging in as Admin and using the dropdown to "Close Account" in the Admin section.

Pharmacompound operates within the legal jurisdiction of the state of California.

Pharmacompound is designed to assist Covered Entities and Business Associates in meeting certain of their obligations under HIPAA. However, compliance with HIPAA is a shared responsibility, and your use of Pharmacompound does not, in and of itself, ensure HIPAA compliance. You represent and warrant that you will handle all Protected Health Information (PHI) in accordance with applicable state and federal laws, including HIPAA.

If you are a Covered Entity or Business Associate, your access to and use of the Pharmacompound service may be subject to a separate Business Associate Agreement. In the event of a conflict between these Terms and the BAA, the terms of the BAA shall supercede and control.

Any PHI will only be used and disclosed as permitted or required by HIPAA, or with the user's authorization. Further, Pharmacompound commits to the "minimum necessary" rule, ensuring that we limit the use and disclosure of PHI to the minimum amount required to accomplish the intended purpose.

Users can exercise their HIPAA rights, such as the right to access, amend, or receive an accounting of disclosures of their PHI.

Pharmacompound uses encryption to protect data in transit (SSL/TLS). Resting data is protected by robust encrypted passwords and Multi-Factor Authentication (MFA).

In the unlikely event of a data breach involving unsecured PHI, Pharmacompound will notify any and all entities affected per the HIPAA Breach Notification Rule.

Pharmacompound may use de-identified data for research, analytics, or other purposes, as HIPAA does not regulate de-identified information. 

These Terms and Conditions may be updated from time to time. We will notify you of any changes by posting the new Terms and Conditions on this page.
